Hours of operation:

Monday and Thursday 1:00 pm - 3:00 pm

Wednesday 3:00 pm - 6:00 pm

Requirement:

-Proof of income for the last 30 days

-Proof of address in Weber County within the last 30 days.

-Photo ID

-Social Security Cards for all members of the household and birth certificates for anyone under the age 18.

-ALL DOCUMENTS ARE REQUIRED AT THE TIME OF SERVICE.
