IN ORDER TO BE SERVED IN THE PANTRY OF THE UNITED METHODIST PUTNAM CITY CHURCH, YOU MUST HAVE THE FOLLOWING:

1.
Social Security Cards: Every family member must have one.

2.
Utility Bill: Zip Code must be one of the following below:
73112, 73122, 73132, 73162, 73120, or 73008 (Bethany)

3.
Diver\'s License OR Photo ID: Each adult must have one.

NOTE: Families will be served 6 times only per year.
